HOW IS CAPRIMIDA INSURANCE AGAINST FEMUR FRACTURE COVERAGE ACTIVATED?
By buying any package of CAPRIMIDA, and keeping your purchase bill, you may call toll-free 800 366 306, provide our operators with the required information and 24 hours after being activated, you will be covered against femur fractures for one month.

HOW TO RENEW THE INSURANCE AGAINST FEMUR FRACTURE? 
The insurance can be renewed for one month with the purchase of any package of CAPRIMIDA by following the above mentioned steps.

HOW TO PROCEED IN CASE OF FEMUR FRACTURE?
-See your Doctor.
-After confirming the femur fracture diagnosis, call 800 366 306 to report the accident.
-Accidents must be reported up to 5 days after taking place.
-The operator will inform the client how to proceed to obtain medical expenses reimbursement.

WHICH ARE THE REQUIREMENTS FOR CLAIM FILING?
-Keeping with you the purchase bill used to activate the insurance against femur fracture.
-Document certifying femur fracture diagnosis.
-Femur fracture X-ray.
-Original receipts of Doctor's fees and prescribed medicines, medical tests and supplies.
-This insurance applies on the expenses resulting after coverage by Isapre/Fonasa and other medical and health insurances.

EXCLUSIONS
-Pre-existing illness and/or fractures; accidents occured under the influence of alcohol or by alcohol, drugs or stimulants abuse; participation in professional bodily contact sports, and/or fractures raised as a consequence of risky sports or activities practice.

SPECIAL CONDITIONS
The following conditions will apply for insurance entitlement:
1.Proving the purchase of products and keeping the relevant purchase bill with you.
2.Being from 18 up to 70 years old.
3.Phoning the above mentioned number and providing full name, ID number, contact telephone number, and the prescribing Physician's details.
4.Keeping record of the activation code informed by the operator.
5.Coverage will come into force 24 hours after activation.
